A remote-access gateway accepted its default password. Inside was an administrator password for the Windows fleet, stored in plain text.

The firewall blocked the direct route. The same password worked over Windows file sharing: 55 of roughly 90 machines, including employee-login servers, software deployment, backups, and code signing. An alert was raised but dismissed as a false positive. Nothing was taken.
